By submitting this form, I approve participation in all anticipated field experiences for my child. Details such as timing clothing and lunch requirements will be communicated closer to the date of the field trip through the teacher's chalkboard.
I give my permission for my child to receive any emergency medical treatment in case of accident. I understand that I am fully responsible for any costs incurred due to negligence or need (medical or otherwise) by and for my child.
In the case of an emergency, attending staff will contact 911 first and then the parent or caregiver.
